Primary Onward Signposting Tool Lite
What is this tool?
Welcome to Penrose Health's new Primary Onward Signposting Tool Lite (or Penrose POST Lite for short!)
As demand for GPs keeps increasing, this tool is designed to ensure we get the right patient to the right place every time. Using a series of questions designed to keep patients safe and understand who can best help them, each POSTcard gives you an output for what to do with the patient's query.
How do I use it?
1. Telephone handshake
A) Greet - answer with a friendly hello, the name of the practice and your name so patients know who they're talking to
B) Identify - data protection is important, so make sure the patient states their name and date of birth. Don't guess!
C) Reason - ask the patient for the reason for their call
P.S. If the caller is calling on behalf of the patient, make sure they are registered to care for the patient!
2. Use Penrose POST
Once you've completed the telephone handshake, identify the most relevant POSTcard. Once you've found the one you think is relevant, follow the questions through to get to an outcome
3. Check and book
When you've got an outcome from the POSTcard, just double-check in your head if it seems reasonable and matches what the patient is telling you. Then simply give the outcome to the patient and book them in on EMIS! Once that's done, please select the answers to the feedback questions - these are critical to us improving the tool for everyone.
