Primary Onward Signposting Tool
What is this tool?
Welcome to Penrose Health's new Primary Onward Signposting Tool (or Penrose POST for short!)
As demand for GPs keeps increasing, this tool is designed to ensure we get the right patient to the right place every time. Using a series of questions designed to keep patients safe and understand who can best help them, each POSTcard gives you an output for what to do with the patient's query.
We're still in testing, so please make sure to pay close attention to what the patient is telling you and use your judgement if you feel the tool is giving the wrong output. We hope that won't happen, but if it does happen, please please please let us know with the feedback at the end of each POSTcard.
How do I use it?
To get started, we've laid out the three basics you'll need to get started:
1. Telephone handshake
Handling calls in a consistent and appropriate manner is essential. When answering any call, please remember the below:
A) Greet - answer with a friendly hello, the name of the practice and your name so patients know who they're talking to
B) Identify - data protection is important, so make sure the patient states their name and date of birth. Don't guess!
C) Reason - ask the patient for the reason for their call
P.S. If the caller is calling on behalf of the patient, make sure they are registered to care for the patient!
2. Use Penrose POST
Once you've completed the telephone handshake, use the blue search bar and identify the most relevant POSTcard. Once you've found the one you think is relevant, follow the questions through to get to an outcome
3. Check and book
When you've got an outcome from the POSTcard, just double check in your head if it seems reasonable and matches what the patient is telling you. Then simply give the outcome to the patient and book them in on EMIS! Once that's done, please select the answers to the feedback questions - these are critical to us improving the tool for everyone.

We have now introduced a coding system for each outcome to help staff trace where the patient came off at and to help improve outcome accuracy.
Simply copy the care name and outcome letter (e.g., Allergies - A) into the booking reason and then enter as much information you have gathered into the booking notes. Please try to avoid using phrases like "F/U" and "See triage".
